Patients in Forfar are being asked to help find a name for their new practice which will open later this year.
The new practice will be formed by the merger of the Academy Medical Centre and Ravenswood Surgery and will be based on the existing site of the Academy Medical Centre in Academy Street.
Anyone who would like to suggest a name should do so in writing to either of the practices, or via e-mail to email@example.com, remembering to add name and phone number. Replies should be submitted by Sunday, February 22 to ensure they are considered.
Members of the Patient Participation Group will then prepare a shortlist which will be published in the Forfar Dispatch and Kirriemuir Herald in early March. The final decision will be by public vote either in person, by e-mailing or writing to the surgeries.
Maxine Jones, project manager for the venture, said: “Planning is well under way for the new practice and patients from both practices have been getting involved in helping us to shape future services.
“We have undertaken a public survey over recent weeks to enable patients to tell us what works well about the care they currently receive from their general practice and where they would like to see improvements made. We would encourage patients interested in helping to shape the care they receive to get involved in the Patient Participation Group.”
Anyone registered with the practices who has comments or suggestions should contact the practice managers or email firstname.lastname@example.org.
